    Place of Birth (District/Country)

3- For Alwah and Talisman

    Your full Name and your mothers name

4- For Books and Magazine

    Write the name of the book or for magazine write month and year.

5- Essential Oil

    Write the name of the Oil.

In Pakistan yearly subscription amount will be send by money order or magazine will be sent to you by VPP.

Other countries personal will send the money through western union.
